E-Statement Opt-In Form

Thank you for your interest in paperless statements. As a Banner Federal Credit Union member, you have the right to have the statements for the accounts you own at Banner Federal Credit Union provided or made available to you through one of the following methods free of charge: paper via postal delivery or electronically through online banking. Maintaining both your paper delivery and electronic delivery of statements will result in a charge of $2.00 per statement delivered by mail per cycle. At anytime you may withdraw this request to have your statements made available online (in lieu of mailed paper statements) by giving us five business days notice in writing. This notice may be sent by first-class mail to us at 9321 W. Thomas Rd Ste 125, Phoenix, AZ 85037 or by contacting us through your Virtual Branch Secure Messaging portal. The recommended method of contacting us is through our online banking Secure Messaging center by selecting Messages.

I hereby request that you make my account statements available for all the accounts I own at Banner Federal Credit Union via Virtual Branch rather than postal mail. Please continue to do so until I withdraw this request as described above.

I understand that the software I must have to view this document via Virtual Branch is Adobe Acrobat. You will need Adobe Acrobat Reader to view the document. If you do not have Adobe you can click here to download a free copy of it. I hereby represent to you that I have the required hardware and software and am able to access the bank's website. Banner Federal Credit Union may change the hardware and software required at any time on 45 days' advance notice. If I choose not to obtain the new hardware or software, I may withdraw my request for e-statements without any fee or charge by you for such withdrawal.

Once you have elected to receive account statements via Virtual Branch, you will be able to print and/or save 24 months of statement. You will also still be able to obtain individual paper statements if needed from our branch offices at the cost of $3.00 per statement.
